Introducing the Safe Steps Framework

Overview

Safe Steps is a clinician-developed, co-produced practice model designed to support mental health staff in responding to consumer distress with consistency, emotional intelligence, and relational integrity. Developed by nurses at Coffs Harbour Acute Mental Health Unit, it was created to meet a frontline need for a structured, real-time approach to managing distress in inpatient settings.

Co-Production and Inclusivity What began as a mental health nurse-led initiative has evolved into a co-produced model shaped by people with lived experience, Aboriginal cultural advisors, and transcultural mental health experts. This inclusive development process ensures that Safe Steps reflects culturally safe, person-centred, and recovery-oriented values.

Purpose and Application

At the core of Safe Steps is a practical, usable framework (Safe Steps) designed for everyday clinical practice - not just theory. It offers clinicians a consistent, repeatable approach to:

  • Support self-regulation and emotional awareness during high-stress interactions
  • Maintain therapeutic rapport and respond early to signs of escalation
  • Avoid reliance on restrictive or reactive measures
  • Provide a calm and structured pathway to support safety and dignity for both consumers and staff

Safe Steps is adaptable to the complexities of real-time ward dynamics and supports professional judgement alongside structured de-escalation strategies.

Research and Impact

As part of a multi-site study, Safe Steps was implemented and evaluated in three acute mental health units, including de-escalation data in over 400 consumers and thousands of clinical episodes. Throughout the 12-month study, nurses used the framework to guide de-escalation and promote safer, more respectful care environments. The research focused not only on practice outcomes but also on nurse reflections and real-time data that highlighted patterns in how distress is managed, and how relational approaches support recovery and safety.

Early findings suggest that Safe Steps supports effective de-escalation, promotes more confident practice, and contributes to positive experiences of care for consumers. These outcomes are being further explored through staff feedback and evaluation of lived experience perspectives.

For nursing and service leadership, Safe Steps offers more than a clinical framework, it provides a scalable, strategic approach to:

Our Services

For First Nations peoples, seclusion and restraint can be highly distressing, traumatising, and re-traumatizing for several reasons. These broadly relate to but are not limited to, colonisation, the stolen generations, discrimination and racism, transgenerational trauma, enforced medical care, and a range of government and non-governmental policies, practices, and experiences over the years. It is imperative that staff deeply acknowledge the impact of this trauma and work to deliver services that are culturally and spiritually safe and trauma-informed.

Safe Steps has been designed using a four-step foundational framework. This structure makes the Steps predictable, easy to use, and repeatable. The framework begins at a foundational level and then builds therapeutic complexity over a number of skill levels, with each level building on the capabilities embedded in the previous level. We have also built a safety checkpoint into the steps. This is the point to check you are safe to proceed - missing this checkpoint may increase the risk of aggression. Although we have devised a stepped structure, we do not view de-escalation as the one-directional deployment of these techniques. We know that skilled mental health nurses will build on the Safe Steps framework and tailor it to the situation.

Safe Steps offers a structured framework for collaborative de-escalation centered on empathy, cultural safety, and respect. Ideally, we want people to leave our services with new understanding and skills that will be helpful in the future. When de-escalation is helpful, it is characterised by transparency and collaboration, with nurses working alongside consumers to explore their problems or difficulties and assist in finding insight, strengths, or resources. Through this process, consumers may discover they can cope with difficult emotions and find control and solutions for their recovery.
